PCP and IPP language, in plain English. A Person-Centered Plan describes what the person wants, what they are good at, and what support they need. The Individual Program Plan then turns those goals into funded services. Copy and paste the language below into a plan, then adjust the names, interests, and goals to fit the person. Person-Centered Plan paragraph: The participant wants to build a small business around a special interest, at their own pace, with one-on-one mentorship. They want support to create a brand, a website, and first customers, without group work, cold-calling, or networking pressure. IPP language, Supported Self-Employment Coaching: individual one-on-one mentorship toward the development and launch of a small business, including brand, website, marketing content, and first sales. IPP language, Continued Supported Self-Employment Coaching: ongoing one-on-one capability-building support for an established small business, including content, customers, scheduling, and invoicing. Measurable outcomes: a live website, active social accounts, professional photos and content, a registered business, a customer agreement template, invoicing and scheduling systems, and at least one real sale by the end of six months. Progress is reported in monthly written summaries, with the participant's consent. A code does not guarantee funding. Funding depends on the IPP goal, Person-Centered Plan, Individual Budget, and Spending Plan, so confirm with an Independent Facilitator or service coordinator. Caliminds Launchpad is an independent provider, not an SDRC vendor, based in Poway and serving all of San Diego County.
